Precision Health USA LLC is committed to protecting the privacy and security of protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state laws.

Our Commitments • We maintain administrative, physical, and technical safeguards for PHI. • We limit access to PHI to workforce members and contractors with a legitimate need. • We use business associate agreements with vendors that handle PHI on our behalf, including telehealth and practice management platforms. • We train staff on privacy and security practices relevant to their roles.

Patient Rights Under HIPAA, you may have the right to: • Access and obtain a copy of certain health records • Request amendments to records in certain circumstances • Request an accounting of certain disclosures • Request restrictions on uses or disclosures in limited situations • Receive confidential communications by alternative means when appropriate • File a complaint with us or with the U.S. Department of Health and Human Services

Notice of Privacy Practices Patients receive a Notice of Privacy Practices describing how PHI may be used and disclosed and how to exercise rights. Intake and portal forms may include acknowledgments related to privacy practices.

Breach Notification If a breach of unsecured PHI occurs, we will notify affected individuals and authorities as required by law.

Security Practices We use encryption for sensitive data, secure authentication for administrative systems, and industry-standard controls for web applications handling patient-related workflows.
